Nightmare (ナイトメア Naitomea) is a horse-like spiritual parasitic demon that, like Piper, is a dangerous being in a most wanted list. When Nightmare enters in one person's dream, the victim will never awake and will have his spiritual energy drained until his demise.

Meiko Rokudō was hired to get hid of the Nightmare, and she asks Reiko Mikami to help her. In the hospital, Mikami believes she is successful, but Nightmare tricked her and goes into her dreams. To save Mikami, Meiko uses Haira so she, Yokoshima, and Okinu can enter in Mikami's mind and defeat the Nightmare. With the Nightmare in control of her mind, Mikami got the form of a large feline and the Nightmare uses her Shadow and four forms of her past to attack. Despite the difficulty, the group manages to help Mikami regain control of her subconscious, leaving Nightmare powerless. Nightmare begs for mercy, but an angry Mikami uses her four past forms to beat him to death.

Later, Nightmare and other beings are revived by Ashtaroth to serve him. He is seen briefly killing people and saying that he will make everyone sleep, but he is crushed by a helicopter.

Nightmare also appears as a boss in the video games Ghost Sweeper Mikami: Joreishi ha Nice Body and Ghost Sweeper Mikami.
